What is Nail-Patella Syndrome?

Nail-Patella Syndrome (NPS) is a rare genetic disorder caused by mutations in the LMX1B gene on chromosome 9. It affects approximately 1 in 50,000 people worldwide. NPS is inherited in an autosomal dominant pattern - meaning a child has a 50% chance of inheriting the condition from an affected parent. Many cases also arise as new, spontaneous mutations.

The LMX1B gene plays a key role in limb and kidney development during fetal life. When mutated, it disrupts the formation of nails, bones, joints and the glomerular basement membrane of the kidneys. The name of the syndrome refers to only two of its features - nails and patellae - but the actual scope of the disease is much broader and affects nearly the entire connective tissue system.

NPS is often called an invisible disability. From the outside most patients look perfectly healthy. Beneath the surface, however, they deal with chronic joint pain, fatigue that never fully goes away and constant uncertainty because the disease can worsen unpredictably. Simple activities - opening jars, climbing stairs, standing for a long time - can become a daily struggle.

The psychological burden is equally real. Living with a condition that most people - including many doctors - have never heard of creates a deep sense of isolation. Patients often feel dismissed, disbelieved or told that their pain is exaggerated.
